Online Payment Option
You can choose to pay your LCDS tuition and/or invoices using our secure online system. All credit card payments are subject to a 2.75% processing fee that is collected by the credit card company. Payments made using the ACH option do not incur any fees.
State & NAEYC Required forms
A current medical form has to be submitted before a child can start the program. Please be aware that no child can be admitted unless we have a current health form on file. If you have any questions please email Tricia
Administration of Medicine
If your child has an allergy and / or needs to have any medication administered at school please contact us as soon as possible that we can ensure that we have everything we need to keep your child safe. Your child will need additional forms to be provided by their pediatrician and / allergist.
Emergency Form & Pick Up Forms
NAEYC requires us to have a form submitted annually providing emergency and pick up information for your child. No child can be admitted without these forms being submitted prior to the first day of school. Please complete these forms and send them to us no later than August 28th.
